Receive personalized, dedicated support with our Concierge Case Management service. This comprehensive program aids in long-term planning, resource connection, and seamless referrals for ongoing support tailored to your unique needs. We provide proactive guidance to navigate challenges and achieve your wellness goals. Access to a specialized Case Manager that advocates, provides assistance in navigating complex systems, connecting to resources and referrals. This is a 2 month (8 weeks) commitment service where you will have weekly check in phone calls to provide status on referrals and resource needs. This includes your initial assessment in home or virtual depending on location and your preference, (1) one weekly phone call at 30 minutes for 2 Months (8 weeks) and Case Manager following up on referrals and coordinating care as needed.
