Case Management Assessment

Service Description

Participate in a thorough assessment to identify your unique needs and challenges. Our experienced case manager will work with you to create a customized plan, connecting you with necessary resources and referrals for ongoing support at home. This is not an ongoing service. You will receive a care plan that you will need to follow up on referrals and community services discussed. If you wish to have follow up we can arrange for follow up phone calls at a rate of $45.00 in 30 minute increments. Schedule for an in home assessment or virtual assessment.
